We’re Upgrading Our Electronic Medical Record System
Our official go-live date with Elation Health is December 8, 2025
Heart of the Valley Pediatric Cardiology and AJM Consultative Medicine are excited to announce that we are transitioning from eMDs to Elation Health, a modern, patient-centered electronic medical record (EMR) platform.
This upgrade reflects our shared commitment to improving the quality, accessibility, and coordination of your care. During this transition, our team will ensure that all patient information is securely transferred, and your care remains uninterrupted.
What This Means for Our Patients
Elation Health provides enhanced tools designed to improve your experience and communication with our care teams:
- New Patient Portal (Elation Passport): Access visit summaries, lab results, and secure messages from your provider.
- Streamlined Communication: Send and receive messages directly with your clinical team.
- Medication and Care Plan Tracking: View active prescriptions, care plans, and physician instructions in real time.
- Secure Sharing of Records: Your medical information can be shared safely with outside specialists and hospitals involved in your care.
Please note that appointment scheduling and cancellations will continue to be managed directly by our office.
We appreciate your patience as we complete this transition. Our staff will assist you with setting up your Elation Passport account and answer any questions you may have.
